Position Overview:

The Occupational Therapist is responsible for delivering therapeutic interventions aimed at addressing physical limitations, functional challenges, or disabilities. This role involves implementing a tailored treatment plan to facilitate recovery, rehabilitation, and restoration of normal bodily functions following an illness or injury. The therapist collaborates with physicians to support the restoration of patient functions, alleviate discomfort, and prevent further complications through the execution of medically prescribed occupational therapy. This position operates with minimal supervision.

Key Responsibilities:

  • Conduct thorough assessments of patients' functional needs and establish suitable objectives in collaboration with the physician; provide direct care, assess outcomes, and consult with other healthcare professionals as necessary.
  • Regularly re-evaluate patients' functional requirements and modify care plans accordingly to ensure the best possible outcomes.
  • Support patients in achieving their treatment goals and adapting to therapeutic devices by administering manual exercises, guiding, motivating, and assisting in physical activities, including non-manual exercises, mobility functions, daily living tasks, and the use of assistive devices.
  • Ensure the continuity of the therapeutic plan by creating home exercise programs; instruct patients, families, and caregivers on these programs; recommend and supply assistive equipment as required.
  • Foster collaboration and communication among various healthcare disciplines within the organization to ensure high-quality patient care.
  • Participate in patient care conferences to promote an interdisciplinary approach to treatment.
  • Document evaluations, treatment goals, and plans promptly in accordance with organizational policies; ensure all documentation is completed accurately and in a timely manner.
  • Manage scheduling effectively to fulfill all assignments and treatments within designated timeframes.
  • Complete scheduled visits and engage in on-call rotations as necessary.
  • Uphold patient rights and dignity throughout the care process.

Additional Duties:

  • Perform other responsibilities as assigned or requested.
  • Adhere to all relevant organizational policies and procedures.
  • Engage actively in continuing education and in-service training.
  • Maintain confidentiality regarding patient information and business practices.
  • Take responsibility for reporting incidents and complaints in line with organizational policy.

Information Management:

Handle all information and data pertinent to the role with the utmost confidentiality and security.

Risk Management:

  • Fully cooperate in all risk management activities and investigations.
  • Stay informed about changes in healthcare regulations.
  • Ensure compliance with local, state, and federal laws, as well as state accreditation standards.

Minimum Qualifications:

Education: Graduate from an accredited Occupational Therapy program.

Experience: A minimum of two years of experience as an occupational therapist within the last five years.

Training: Completion of necessary competency assessments.

Licensure/Certification: Current licensure; eligibility for or successful completion of the entry-level certification examination for occupational therapists (NBCOT); CPR certification; valid driver's license and proof of auto liability insurance.

Work Environment:

Position involves working in patients' homes under various conditions, with potential exposure to blood, bodily fluids, and infectious diseases. Flexibility in scheduling and local travel is required, along with some exposure to adverse weather conditions. The role may involve moderate noise levels and tasks that can be stressful and emotionally demanding.

Physical Requirements:

Prolonged periods of standing and walking are necessary. The ability to lift up to 50 lbs. and assist patients is required. The role demands working under pressure to meet deadlines, identify patient needs, make swift decisions, and address psychosocial needs of patients and families. Proficiency in using durable medical equipment in home settings is essential.
