Responsibilities- Provide administrative support to the nursing team, including answering phone calls, responding to emails, and maintaining accurate records.
- Assist with the coordination of patient care, including scheduling appointments, ordering supplies, and communicating with healthcare professionals.
- Support the development and implementation of patient care plans, working closely with the interdisciplinary team to ensure seamless care transitions.
- Collaborate with the nursing team to identify and address patient needs, providing emotional support and comfort to residents and their families.
- Graduate of an approved LPN program.
- Experience in Skilled Nursing/Long Term Care, with a focus on post-acute care.
- LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required.
- CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required.
