Clinical Nurse Long Term Care Specialist

We are seeking a highly skilled and compassionate Clinical Nurse to join our team in the Post Acute Care Units at Corewell Health. As a Clinical Nurse, you will play a vital role in providing exceptional patient care and contributing to the well-being of our residents.
Key Responsibilities- Utilize the nursing process to provide high-quality patient care, incorporating patient-family centered care principles and upholding Corewell Health's mission and vision.
- Work collaboratively with a multidisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our residents.
- Provide education and support to patients, residents, and families, promoting empowerment and self-care.
- Participate in ongoing education and professional development, staying current with best practices and evidence-based care.
- Contribute to a positive and respectful work environment, fostering open communication and teamwork among colleagues.
- Associate's Degree or equivalent ADN
- Experience in Skilled Nursing/Long Term Care
- LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
- CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
