We are seeking a highly skilled and compassionate Palliative Care Clinical Nurse Specialist to join our community team at Hampshire Hospitals NHS Foundation Trust.
Key Responsibilities- Provide high-quality palliative and end-of-life care to patients in their own homes, working collaboratively with the community team.
- Develop and deliver education to healthcare professionals, patients, and their families on palliative care.
- Contribute to the development and delivery of the community palliative care service, working in collaboration with the multidisciplinary team.
- Work autonomously and as part of a team to provide symptom management and psychological and social support to patients and their families.
- Professional UK registration in NMC.
- Evidence of working with palliative care patients and acute hospital experience.
