Job Summary

We are seeking a highly skilled Clinical Practice Leader to join our team at Health and Wellness Providers (HWP). As a key member of our clinical leadership team, you will be responsible for leading all clinical activities at our practice, ensuring the delivery of high-quality care to our patients.

HWP is a innovative healthcare provider focused on providing chronic care management (CCM) and remote patient monitoring (RPM) services to patients through our novel service offering. As a Clinical Practice Leader, you will play a critical role in building and leading our clinical team, working closely with Senior Management to develop and implement our clinical strategy.

Key Responsibilities:

  • Oversee the clinical activities at the practice, ensuring the delivery of high-quality care to patients
  • Create, update, and maintain clinical protocols to be used by staff members and/or contracted care providers
  • Work with the clinical team to maintain patient comprehensive care plans, including problem lists, treatment goals, and medication management
  • Provide coaching and guidance to the clinical team to deliver high-quality care
  • Implement enhancements and improvements in the clinical practice to provide the best care to patients
  • Utilize our specialized CCM/RPM platform and eHR software to properly document interactions
  • Partner with the patient's care team to drive clinical outcomes, including reducing admissions and hospitalizations
  • Provide medical decision-making for the practice, including direct interventions for patients
  • Participate in continuing quality improvement activities for all staff, including identifying and resolving opportunities for staff education and process improvements

Requirements:

  • Current, valid MA Licensed Certified Nurse Practitioner (NP) or Certified Nurse Specialist (CNS) with independent practice authority in MA
  • 2+ years' experience in a primary care/internal medicine role working with patients with chronic disease states
  • Creativity to drive rapid change and improvement in HWP's service offering to patients
  • Experience with delivering care to patients virtually preferred
