Key Responsibilities:- Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
- Coordinate patient care activities, including scheduling, testing, and referrals
- Provide patient and family education on medical conditions, treatment regimens, and self-care
- Ensure patient-centered care, incorporating individual needs and preferences
- Registered Nurse with a Bachelor's Degree in Nursing (BSN) preferred
- At least 2-3 years of experience in a pediatric inpatient or outpatient setting
- Excellent interpersonal and organizational skills
- Ability to assert oneself, advocate for patients, and work independently with minimal supervision
- Computer experience and knowledge of word processing and data entry systems
- Accountable for following hospital policy and procedures
- Accountable for conducting appropriate coordination of care for patients
- Evaluates and coordinates care for multiple types of medically complex pediatric patient populations
- Must be self-motivated with the ability to multi-task, prioritize, and initiate processes to resolve problems
- Excellent interpersonal and organizational skills
- Ability to assert oneself, advocate for patients, and work independently with minimal supervision
- Ability to manage multiple priorities
- Computer experience and knowledge of word processing and data entry systems
