Key Responsibilities:
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
  • Coordinate patient care activities, including scheduling, testing, and referrals
  • Provide patient and family education on medical conditions, treatment regimens, and self-care
  • Ensure patient-centered care, incorporating individual needs and preferences
Key Qualifications:
  • Registered Nurse with a Bachelor's Degree in Nursing (BSN) preferred
  • At least 2-3 years of experience in a pediatric inpatient or outpatient setting
  • Excellent interpersonal and organizational skills
  • Ability to assert oneself, advocate for patients, and work independently with minimal supervision
  • Computer experience and knowledge of word processing and data entry systems
Accountability and Complexity:
  • Accountable for following hospital policy and procedures
  • Accountable for conducting appropriate coordination of care for patients
  • Evaluates and coordinates care for multiple types of medically complex pediatric patient populations
  • Must be self-motivated with the ability to multi-task, prioritize, and initiate processes to resolve problems
Additional Requirements:
  • Excellent interpersonal and organizational skills
  • Ability to assert oneself, advocate for patients, and work independently with minimal supervision
  • Ability to manage multiple priorities
  • Computer experience and knowledge of word processing and data entry systems
