Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Private Duty Care Specialist. As an LPN with IntelliChoice Home Care, you will have the opportunity to work one-on-one with patients, providing critical care in the home setting.

Responsibilities
  • Provide one-on-one nursing care to patients in their homes, focusing on tracheostomy and/or ventilator patients of all ages.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with our respiratory therapists to provide comprehensive care and support to patients and families.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Current LPN license in good standing.
  • CPR certification (in-person training only).
  • TB skin test (current within the last 12 months).
