Job Summary

We are seeking a skilled Licensed Practical Nurse to join our interdisciplinary team. As a key member of our care team, you will participate in reviewing, planning, coordinating, and evaluating resident care. Your expertise will be essential in documenting resident conditions and nursing needs accurately and in a timely manner.

Key Responsibilities
  • Participate in interdisciplinary team meetings to review and plan resident care
  • Document resident conditions and nursing needs accurately and in a timely manner
  • Develop, direct, and monitor nursing assistant assignments
  • Execute treatments as necessary and document reactions to medications and treatments
  • Generate, prepare, administer, and chart medications
  • Initiate physician orders
  • Facilitate communication with families regarding changes in medications and/or resident care
  • Establish protocols to ensure care plans are continually reviewed for updated and accurate data
  • Coordinate admissions, discharges, and transfers to deliver quality customer service
Requirements
  • Licensed Practical Nurse graduate from an accredited nursing program
  • Certifications and Licensed Practical Nurse license and other licensure required by state regulations
