Job Summary

Element Care is seeking a highly skilled Physician to join our team as a Geriatric Care Specialist. As a key member of our PACE (Program of All-Inclusive Care for the Elderly) team, you will be responsible for providing high-quality medical care to our geriatric population.

Responsibilities
  • Conduct initial, periodic, and annual physical examinations of participants.
  • Evaluate participants for episodic visits, acute illnesses, and follow-ups of hospitalizations in the clinic, at home, or at a nursing home.
  • Make nursing home visits in accordance with state nursing home requirements and as needed.
  • Utilize telehealth services as needed and where appropriate.
  • Participate in the PACE hospital rounding system and on-call schedule.
  • Evaluate participant needs and develop treatment plans in concert with the Interdisciplinary Team.
  • Provide culturally competent healthcare and education to patients, their families, and caregivers, as well as to other members of the Interdisciplinary Team.
Qualifications
  • Demonstrated ability to work within urban communities, particularly with racially, ethnically, and culturally diverse, low-income, and special needs populations.
  • Board Certified in Internal Medicine or Family Practice, or eligibility for Board Certification (for recent graduates).
  • Board Certified in Geriatric Medicine, preferred.
  • Current License to practice medicine in the Commonwealth of Massachusetts.
  • Current Massachusetts and DEA Prescription Privileges. Suboxone Waiver preferred.
  • Maintains appropriate hospital and long-term care facility credentials and privileges.
  • Current CPR Certification.
  • At least one (1) year of experience working with a geriatric population.
  • Ability to relate well to participants, anticipate their needs, and encourage their independence.
  • Preferred: At least one (1) year of experience working with low-income and diverse cultural groups.
  • Ability to convey information clearly and succinctly with team members.
  • Ability to work well with others and be a dedicated team member.
  • Ability to pay attention to detail, be a self-starter, and complete assignments on time with minimal direction.
  • Ability to quickly recognize situations/problems before they become acute and implement effective solutions and understand consequences.
