Job Description

We are seeking a skilled MDS Coordinator to join our team at ExcelCare At the Pines. As a critical member of our interdisciplinary team, you will play a vital role in ensuring the highest quality care for our residents.

Key Responsibilities:
  • Conduct accurate and timely MDS assessments in accordance with RAI coding guidelines
  • Develop and implement individualized care plans that meet the unique needs of each resident
  • Collaborate with the interdisciplinary team to ensure seamless coordination of care
  • Monitor and audit clinical records to ensure accuracy and timeliness
Requirements:
  • Valid RN license
  • Prior experience with MDS 3.0 is required
  • Previous long-term care experience is essential
  • Ability to work 40 hours per week
