About the Role

Optum, a leading healthcare organization, is seeking a skilled Licensed Practical/Vocational Nurse to join our team as a Home Health Care Specialist. As a key member of our care delivery team, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Responsibilities
  • Direct Patient Care: Provide direct patient care under the supervision of a Registered Nurse, following the plan of care and implementing treatments as defined in the state Nurse Practice Act and agency's policies.
  • Plan of Care Implementation: Implement the plan of care initiated by the Registered Nurse, ensuring accurate and timely documentation consistent with the plan of care.
  • Patient Education: Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Team Collaboration: Participate in the coordination of home health services, reporting identified needs for other disciplines to the Registered Nurse and/or Clinical Supervisor.
  • Infection Control: Follow all infection control standard precautions and safety guidelines/standards as per agency policy.
  • Equipment and Supplies: Use equipment and supplies effectively and efficiently.
  • Personal and Professional Growth: Participate in personal and professional growth and development.
  • Documentation: Adhere to the Agency's Standard Operating Procedures as it relates to the submission of documentation.
Communication
  • Clinical Notes: Prepare clinical notes and update the primary physician when necessary.
  • Physician Communication: Communicate with the physician regarding the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders as required.
  • Service Coordination: Coordinate services and schedules with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC/Scheduler) to include recommendations for additional home health care services for patients by the end of the work day.
Additional Duties
  • On-Call Duties: Participate in on-call duties within the on-call rotation schedule, to include weekends as assigned.
  • Patient Rights: Demonstrate knowledge and observance of Patient Rights and Notice of Privacy Practices.
  • Equipment and Supplies: Ensure that arrangements for equipment and other necessary items and services are available.
  • Case Load and Documentation: Maintain a daily patient case load and point of care documentation levels, as per Agency standards.
  • Availability: Require the availability of accepting patients during business hours (8 a.m. - 6 p.m.).
  • Time Card: Require updating the time card daily, as per UHG Policy.
  • Pre-Approval: Require obtaining pre-approval from direct supervisor for overtime.
  • Flexibility: Require the ability to work flexible schedule to meet patients' needs.
  • Attendance and Punctuality: Demonstrate personal responsibility with regard to attendance and punctuality.
  • Confidentiality: Maintain privacy and confidentiality with regard to all patient, staff, and agency information.
  • Teamwork: Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others or in place of others, as necessary.
  • Communication: Express verbal and written communication in a clear, positive, and cooperative manner.
  • Agency Image: Promote the agency's image by adhering to the agency's dress code.
  • Additional Responsibilities: Perform all other related duties as assigned.
