Physical Therapist

We are seeking a highly skilled Physical Therapist to join our PACE (Program of All-Inclusive Care for the Elderly) team at UnityPoint Health. As a Physical Therapist, you will play a vital role in evaluating and treating patients requiring physical therapy services, working collaboratively with our interdisciplinary team to deliver high-quality patient care.
Key Responsibilities- Perform thorough examinations and evaluations to determine patient diagnoses and prognoses, and develop effective treatment plans to achieve desired outcomes.
- Utilize a range of physical therapy interventions to produce optimal results, including modalities and administrative tasks.
- Communicate patient progress and status to the PACE interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals, to ensure seamless care coordination.
- Maintain accurate and detailed documentation of patient care, adhering to state licensure laws and hospital policies.
- Supervise Physical Therapist Assistants as needed, ensuring compliance with state licensure laws and hospital procedures.
- Participate in quality assurance programs and meetings to ensure high-quality patient care and continuous improvement.
- Graduate of an accredited physical therapy program.
- Valid driver's license and auto insurance.
- Licensed by the state of Iowa.
- Ability to work effectively within an interdisciplinary team environment.
