Job Description

At RVNAhealth, we are committed to providing exceptional care to our patients and their families. As a Hospice RN - Continuous Care Specialist, you will play a vital role in delivering high-quality patient care in accordance with physicians' orders.

Key Responsibilities:

  • Provide direct patient care as assigned, reporting all pertinent findings to the RN Case Manager, physician, and Clinical Manager or designee.
  • Deliver palliative nursing care to patients and families, incorporating psychosocial, cultural, physical, spiritual, and biological components and appropriate nursing intervention as identified in the plan of care developed by the IDG.
  • Consult and collaborate with Hospice IDG to promote quality and continuity of patient/family care.
  • Document observations/interventions to support higher level of care.
  • Attend patient deaths; prepare body for transport as needed, educate family on drug disposal methods.
  • Provide bereavement support to the patient/caregiver family.

Requirements:

  • Graduate of an accredited school of nursing.
  • Currently licensed as a Registered Nurse in the State of Connecticut.
  • A minimum of two years of clinical experience.
  • Preferred, minimum one year of hospice or home care experience with a strong knowledge base of community care and associated regulations.
