Welcome to the Southcentral Foundation Community Case Manager role. As part of our team, you will be responsible for providing support services to customer-owners, assisting with community resources, and implementing service plans to enhance daily life.

At the Adult Outpatient Services Behavioral Health Clinic, we focus on trauma-informed, systems-based, and person-centered approaches to serving adults aged 18 and older. Our team offers individual, group, and couples therapy tailored to customer needs and clinical expertise. Versatility in addressing a wide range of diagnoses is essential as you collaborate with a multidisciplinary team.

Our programs primarily serve Alaska Native and American Indian people within our geographical area. It is crucial for employees to have a deep understanding of these cultures to support SCF's vision and mission of promoting wellness.

Key Responsibilities:

  • Provide support services to customer-owners, including assistance with community resources and service plan implementation.
  • Collaborate with a multidisciplinary team to address a wide range of diagnoses.
  • Develop and implement individualized service plans to enhance daily life.
  • Assist with community outreach and engagement efforts.

Qualifications:

  • BA/BS degree in a social services field and one year of direct care experience in a behavioral health setting, or equivalent combination of education and experience.
  • Additional year of professional social service experience or equivalent combination of education and experience, or demonstrated proficiency as a Behavioral Health Case Manager/Behavioral Health Technician II at SCF.
  • Must meet specific conditions to qualify as an approved SCF driver.

Employee Health Requirements:

Compliance with the Employee Health Procedure is mandatory for SCF employment. All job-related health screenings and immunizations must be completed before the first day of employment, especially for Health Care Personnel positions.
