Job Summary:

Signature HealthCARE LLC is seeking a highly skilled and experienced MDS Coordinator to join our team. As a key member of our healthcare operations team, the MDS Coordinator will play a critical role in ensuring the highest quality of care for our residents.

Key Responsibilities:

  • Process Improvement: Identify and participate in initiatives that enhance the customer experience, improve workflow, and optimize the work environment.
  • Medicare Compliance: Lead or participate in daily PPS meetings, weekly Medicare meetings, and monthly meetings to ensure federal billing requirements are met.
  • Resident Assessment: Oversee the coordination and completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in accordance with current Federal and State Regulations.
  • Interdisciplinary Team Collaboration: Assist in the completion of the Resident Assessment Instrument with the Interdisciplinary Team.

Requirements:

  • Education and Licensure: Registered Nurse or Licensed Practical Nurse with current state licensure.
  • Experience: Minimum of one (1) year MDS Coordinator experience in the long-term care setting.
  • Certifications: Current/active CPR certification.
  • Personal Qualities: Personal integrity and professionalism to work effectively with the Interdisciplinary Team, patients, and families.
  • Knowledge and Skills: Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
