Job Summary

The Field Nurse will provide professional supervision and on-the-job training to personal care aides, assist with clinical orientation of newly hired paraprofessional staff, and provide medical oversight for clients in their homes. The ideal candidate will have a strong understanding of the scope of practice of professional and paraprofessional healthcare, excellent organizational and communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities
  • Provide in-field professional supervision and on-the-job training to personal care aides.
  • Assist with clinical orientation of newly hired paraprofessional staff.
  • Provide medical oversight for clients in their homes, including assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care.
  • Maintain current DOH regulations regarding 6-month recertifications and Post Hospital visits.
Requirements
  • NYS licensed and currently registered as a Registered Professional Nurse.
  • Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to speak effectively before customers/patients or employees of the agency.
  • Excellent organizational, writing, communication, and interpersonal skills.
  • Ability to apply common sense understanding to carry out instructions furnished in written, verbal, or diagram form.
  • Home Care experience is required.
  • HHAeXchange software knowledge is a plus but not required.

Pay: $40.00-$50.00

License/Certification: RN (Required), Driver's License (Required)

Work Location: Combined 80% Field & 20% Office/Home for documentation.
