Job Summary

This position is responsible for facilitating the placements of program participants upon release from designated correctional institutions to community-based programs. The successful candidate will collaborate with parole officers, transportation coordinators, and community-based providers to ensure a smooth transition for participants.

Key Responsibilities

  • Collaborate with parole officers to determine placement and ensure a seamless transition for participants.
  • Work with transportation coordinators to arrange transportation to community-based programs.
  • Assist community service representatives in determining placement and reception into programs.
  • Track and monitor referrals from parole officers and conduct assessments to determine participant needs.
  • Collaborate with community-based providers to determine reception into programs and ensure participants have necessary medication and supplies.
  • Respond to agent concerns and communicate effectively with correctional institutions.
  • Complete required reports as required by the program manager and correctional institutions.

Requirements

  • High school diploma or equivalent.
  • Experience with case management and criminal justice populations is preferred.
  • Ability to understand various models, including cognitive behavioral therapy, and theories of addiction.
  • Knowledge of the California Treatment Initiative and the transitional process between in-custody and community-based modalities.
  • Excellent communication and organizational skills.
  • Ability to read and interpret documents, write routine reports, and establish rapport with participants.
