Hope At Home Health Care offers a rewarding opportunity for an experienced Physical Therapist to join our team in providing high-quality patient care. We are seeking a compassionate and skilled professional to work as a PRN-Physical Therapist.

About the Job:

We are looking for a dedicated individual with excellent interpersonal skills and a current state PT license to provide medical support to our patients while ensuring great customer service.

Responsibilities:

  • Provide physical therapy services to patients according to a written physician's plan of care, which may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care.
  • Evaluate home environment and assess muscle strength, mobility, gait, ROM—potential for rehab.
  • Direct physical therapy treatment and instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report to physicians patient's reaction to treatment or changes in condition.

Initiating Physical Therapy Programs:

  • Develop and implement physical therapy programs and instruct other personnel and/or family/caregivers in certain phases of physical therapy.
  • Train patients in the use of prosthetic devices and identify patient and family/caregiver needs for other home health services, referring as necessary.

Qualifications:

  • Possess a degree from a program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the state.
  • Two years of appropriate experience as a physical therapist, preferably in community/home health settings.
  • Demonstrate good verbal and written communication and organizational skills.
  • Possess and maintain current CPR certification.

Our Benefits:

We offer a competitive salary range of $85,000-$110,000 per year, depending on experience, as well as opportunities for professional growth and development.
