About the Role

We are seeking a skilled Rehabilitation Therapist Specialist to provide high-quality medical support to our patients in their homes.

Job Responsibilities

The ideal candidate will have excellent communication skills and be able to work independently as part of our team. Key responsibilities include:

  • Assessing and developing individualized plans of care for patients.
  • Providing physical therapy services according to written physician's orders, including evaluating therapeutic/rehabilitative/functional status.
  • Directing physical therapy treatment, instructing patients and families/caregivers on appliance use, and determining priority needs for physical therapy.
  • Initiating programs, instructing other personnel, and identifying patient needs for other home health services.
Requirements and Qualifications

To succeed in this role, you must possess:

  • A degree from a program in physical therapy approved by an accredited organization.
  • Licensure to practice as a physical therapist within the state.
  • At least two years of experience as a physical therapist, with a focus on community/home health settings.
  • Excellent verbal and written communication, organizational, and interpersonal skills.
Compensation and Benefits

We offer a competitive salary range estimated at $60,000 - $80,000 annually, based on experience, plus benefits that prioritize your well-being and career growth.
