Job Summary:
As a Hospice Care Specialist with LifeBridge Health, you will play a vital role in ensuring that all services are delivered in accordance with our Mission Statement and S.P.I.R.I.T. Values. You will work as a para-professional, providing personal care and other health care needs of patients under the direction of a Registered Nurse.
Key Responsibilities:
- Deliver high-quality patient care and support services in accordance with established standards and protocols.
- Work collaboratively with the healthcare team to ensure seamless care coordination and communication.
- Provide emotional support and comfort to patients and their families, promoting a positive and compassionate care environment.
- Participate in ongoing education and training to enhance skills and knowledge in hospice care and patient support.
Requirements:
- High School Diploma or equivalent required.
- Successful completion of a Certified Nursing Assistant (CNA) course required.
- Previous experience as a CNA/Home Health Aide, preferably in a Medicare Certified Program, preferred.
- CNA - Certified Nursing Assistant certification required.
- CPR - AHA Healthcare Provider certification required.
- CHPNA - Certified Hospice Palliative Nurse Assistant certification preferred.
