**Job Description:**
We are seeking a skilled Registered Nurse with experience in chronic or home hemodialysis to participate in the Home Therapies Program. As a member of our nephrology healthcare team, you will be responsible for decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and program operations.
You will identify and evaluate candidates for the Home Therapies Program, assess patients' home environments, and perform assessments and identifies barriers of the Home Dialysis candidate's home environment and caretaker readiness and ability to perform dialysis treatments in the home.
In this role, you will coordinate patient care, including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients. You will analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed.
As a Patient Care Specialist - Dialysis Education, you will provide initial and ongoing education to patients and families on home dialysis treatment and the operation and maintenance of all home therapies equipment. You will ensure patient awareness related to transplant and treatment modality options and administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness.

**Required Skills and Qualifications:*
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
*While experience as a Registered Nurse and chronic/home hemodialysis experience are preferred, they are not required. Successful applicants must pass the Ishihara Color Blind Test and demonstrate compassion, caring, support for colleagues, collaborative, reliable qualities and traits.
