About the Position

This part-time position involves the evaluation of patient psychosocial needs and the provision of diagnostic/preventive and treatment services. You will work closely with other healthcare professionals to ensure that patients receive comprehensive care. We are seeking candidates with strong communication skills, ability to multitask in a fast-paced environment, and knowledge of community resources and systems. Experience in a healthcare or mental health setting is preferred.

Responsibilities

• Evaluate patient psychosocial needs and develop individualized treatment plans
• Provide complex discharge planning and coordinate aftercare services
• Maintain accurate records of patient interactions and treatment progress

Requirements

To be successful in this role, you will need to possess a Master's Degree in Social Work/Clinical Counseling from an institution accredited by the Council on Social Work/Counselor Education, as well as current licensure as an LCSW (Licensed Clinical Social Worker) or LMSW-CC (Licensed Master Social Worker, Conditional Clinical). Two years of relevant experience is preferred.

Salary Range: $65,000 - $85,000 per year
