We are seeking a highly skilled and compassionate RN to join our team as a Care Coordinator RN.
Key Responsibilities- Provide advanced clinical home care services, including patient assessment, medication management, and patient/family education.
- Assume primary responsibility for coordinating, implementing, and evaluating the home care needs of patients.
- Review patient insurance information and coordinate activities involved in each individual care plan.
- Share pertinent findings with family and professional caregivers.
- Current NYS RN license and registration required.
- Bachelor's degree in nursing preferred; Associate's degree required.
- Minimum of one year acute medical-surgical nursing experience required.
- Certified Home Health Agency (CHHA) experience preferred.
- Bilingual English and Spanish or Mandarin or Cantonese preferred.
