About the Role

We are seeking a skilled Clinical Care Specialist to join our team at May Seven Home Health Llc. As a Clinical Care Specialist, you will be responsible for delivering high-quality patient care in the comfort of clients' homes.

Key Responsibilities
  • Conduct comprehensive assessments for new home care clients.
  • Develop and implement personalized patient care plans.
  • Monitor patients' symptoms and behavior, reporting any changes promptly.
  • Complete discharge forms when clients are no longer in need of our services.
  • Educate patients on maintaining their health and preventing diseases.
  • Perform regular visits to ensure compliance with agency standards.
Requirements
  • Prior experience in nursing or related medical fields.
  • Ability to build strong professional relationships with clients and colleagues.
  • Excellent problem-solving and critical thinking skills.
  • Capacity to thrive in a fast-paced environment.
Compensation Package

The estimated salary for this role is $65,000-$85,000 per year, depending on experience and qualifications.

Maintenance of certification and ongoing education opportunities are also provided.
