Job Summary

We are seeking a highly skilled Inpatient Case Manager to join our team at Kaiser Permanente. As a key member of our healthcare team, you will be responsible for working collaboratively with physician partners to optimize quality and efficiency of care for hospitalized members. Your primary focus will be on carrying out daily utilization and quality review, monitoring for inefficiencies and opportunities to improve care, and developing a safe discharge plan to include recommending alternative levels and sites of care when appropriate.

Key Responsibilities
  • Conduct daily review of hospital care by chart review and discussion with attending physician to identify areas for improvement.
  • Develop and implement a discharge plan to meet each patient's identified needs, ensuring a seamless transition from one level of care to another across the continuum.
  • Collaborate with multidisciplinary healthcare team to develop, implement, and document individualized plans of care to ensure continuity, quality, and appropriate resource use.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure timely and efficient delivery of essential services.
  • Monitor and evaluate patient progress, identifying opportunities for improvement and implementing changes as needed.
Requirements
  • Minimum two years of RN experience in patient care delivery or completion of Masters degree in Case Management Program in lieu of minimum years of experience.
  • Associates Degree in Nursing.
  • Registered Professional Nurse License (Georgia).
Preferred Qualifications
  • Minimum five years of clinical nursing experience in a hospital setting.
  • Minimum five years of professional practice experience in an acute care setting.
  • Minimum two years of experience in utilization review, case management, and discharge planning.
