Job Summary

We are seeking a skilled Patient Care Navigator to join our team at Kaiser Permanente in San Rafael. As a Medical Social Worker II, you will play a critical role in ensuring that patients and their families receive comprehensive care and support.

About the Role

In this position, you will work closely with patients, families, and healthcare teams to assess and address social, emotional, and practical needs related to illness or disability. Your expertise will help individuals navigate the complexities of healthcare, access community resources, and develop personalized plans for care.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective treatment plans
  • Collaborate with multidisciplinary teams to coordinate care, communicate effectively, and ensure seamless transitions
  • Provide counseling, education, and support to patients and families on disease management, coping strategies, and resource utilization
  • Facilitate connections to community resources, government programs, and external services to enhance patient outcomes
  • Maintain accurate and timely documentation, records, and data collection to inform care decisions
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work from an accredited program
  • At least 2 years of experience in social work, preferably in a healthcare setting
  • Strong assessment, counseling, and communication skills
  • Ability to work independently and collaboratively as part of a dynamic team
  • Proficiency in automated clinical information systems and computer-literate
What We Offer

Kaiser Permanente offers a competitive salary range of $85,000 - $110,000, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, retirement savings plan, and paid time off.
