Job Summary

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Visits Specialist. As a Physical Therapist, you will be responsible for providing rehabilitative care and skilled nursing to clients in their homes, promoting their independence and quality of life.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop individualized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement evidence-based interventions to prevent wound formation and promote wound repair.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function.
  • Educate clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art electronic medical record system.
Qualifications
  • Current Pennsylvania Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
