Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement manual therapeutic exercises, gait training, and other interventions to improve clients' functional abilities.
  • Collaborate with nursing staff to develop and revise care plans, ensuring seamless multidisciplinary care.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
Requirements
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
