Job Summary

We are seeking a skilled Physical Therapist to join our team at Interim HealthCare Home Health and Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized plans of care to meet the unique needs of each patient.
  • Provide skilled physical therapy services, including exercises, modalities, and education, to promote optimal patient outcomes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
  • Contribute to the growth and development of our team by sharing knowledge and expertise.

Requirements

  • Current licensure as a physical therapist in the state of Michigan.
  • At least one year of experience as a physical therapist in the last five years.
  • Active CPR certification.
  • Previous home health experience preferred.
  • Reliable transportation with a valid state driver's license and car insurance.
