We are seeking a skilled Physical Therapist to join our team at Interim HealthCare Home Health and Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to our mission of delivering exceptional healthcare services.

About the Role

This part-time position offers a unique opportunity to work with patients in their homes, providing personalized care and support. As a Physical Therapist, you will be responsible for:

  • Conducting comprehensive assessments and developing individualized plans of care
  • Providing services ordered by physicians, including physical therapy treatments and interventions
  • Participating in interdisciplinary team meetings and care conferences
  • Collaborating with patients, families, and healthcare professionals to achieve optimal outcomes
Requirements

To be successful in this role, you will need:

  • Current licensure as a physical therapist in the state of Michigan
  • At least one year of experience as a physical therapist in the last five years
  • Active CPR certification
  • Previous home health experience is preferred, but not required
  • Reliable transportation and a valid state driver's license
