POSITION SUMMARY:
  • Oversees the integration and collaboration of social services within hospital and community settings, ensuring that patients, families, medical personnel, and community organizations benefit from effective social work practices.
  • Delivers services aligned with the core mission, vision, and values of Hancock Health.
  • Undertakes additional responsibilities as required.
QUALIFICATIONS:

KEY FUNCTIONS AND RESPONSIBILITIES:
  • Conducts comprehensive Biopsychosocial Assessments for patients to identify Social Determinants of Health (SDOH) and facilitates referrals to appropriate community resources.
  • Performs Psychiatric Risk Assessments for individuals at risk, determining necessary levels of care and collaborating with medical staff for psychiatric placements as needed.
  • Provides Crisis Intervention and employs de-escalation techniques to manage urgent situations.
  • Coordinates transportation services for patients requiring assistance from the Emergency Department, utilizing resources such as rideshare services.
  • Maintains an extensive knowledge of referral resources in the service area and fosters positive relationships with relevant community organizations.
  • Supports various inpatient units by providing essential services and resources, including assistance with Substance Misuse Treatment, discharge planning, and support for families facing loss.
  • Offers support within the Emergency Department concerning social services.
  • Implements a data tracking system to monitor referrals and services rendered.
  • Facilitates educational sessions for healthcare team members regarding the social and emotional aspects of health challenges.
  • Enhances the social service program through the continuous development and revision of policies and procedures.
  • Engages in Quality Improvement initiatives as identified.
  • Ensures accurate and timely documentation within the electronic medical record system in accordance with departmental policies.
  • Represents the hospital in meetings and committees focused on addressing patients' social needs.
  • Mentors and supervises Social Work students.
  • Adheres to the ethical standards of Hancock Regional Hospital, exemplifying the organization's values.

EDUCATION AND EXPERIENCE REQUIREMENTS:
  • Master's Degree in social work or a related discipline. Must be licensed or eligible for licensure as LSW or LBSW.
  • Stays informed on psychosocial developmental issues and regulatory requirements through ongoing education and professional development.
