Job Title: MDS Coordinator RN West Saint Paul

Walker Methodist Westwood Ridge in West St. Paul is seeking a skilled MDS Coordinator RN to join our team. As a key member of our Transitional Care Unit (TCU), you will play a vital role in ensuring the highest quality care for our residents.

Responsibilities:
  • Monitor and review resident charts to ensure accurate and timely completion of Minimum Data Set (MDS) assessments.
  • Collaborate with the Interdisciplinary Team (IDT) to ensure accurate and complete Electronic Medical Records (EMRs).
  • Act as a primary trainer for the MDS and care planning process, providing education and support to new staff members.
  • Assist in the completion of MDSs as assigned by supervisor.
Requirements:
  • Current experience with MDS/CAA completion, care planning, transmissions, and error follow-up.
  • Current experience with Medicare/PDPM requirements.
  • ICD-10 coding experience.
  • Ability to work with corporate finance team and insurance carriers on billing issues, appeals, and insurance claims.
Preferred Qualifications:
  • Graduate of an RN/LPN accredited school of nursing with a current active RN/LPN license in the State of Minnesota.
  • Previous long-term care experience.
  • Current experience with Medicare PPS.
