\t* Assess and evaluate patients to develop and implement individualized plans of care
\t* Collaborate with pharmacy staff, physicians, and patients to develop and monitor treatment goals
\t* Manage vascular access devices, including insertion of peripheral infusion devices or peripherally inserted catheters (if certified)
\t* Educate patients and caregivers on infusion therapy and related topics
\t* Evaluate progress toward goals and make recommendations for changes in the plan of care as needed
\t* Coordinate nursing care with home health and other community agencies as indicated
\t* Maintain accurate and detailed records of patient care and treatment
\t* Participate in quality improvement initiatives and contribute to the development of policies and procedures
\t* Collaborate with other healthcare professionals to ensure seamless patient care
\t* Maintain current knowledge of infusion therapy and related topics through ongoing education and training
\t* Adhere to all relevant laws, regulations, and standards of practice
\t* Maintain confidentiality and handle sensitive patient information with discretion
\t* Perform other duties as assigned by management

\t* Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred
\t* Valid nursing license in state(s) of practice
\t* Minimum of one (1) year of experience as a Registered Nurse
\t* Previous infusion and home health care experience preferred
\t* Extensive experience in IV therapy preferred
\t* Current CPR certification
\t* Flexible schedule with the availability to work a variety of shift models
\t* Ability to work effectively within a multidisciplinary team
\t* Effective verbal and written communication skills
\t* Valid driver's license and auto insurance
\t* Must meet company standards for driving record as well as driver age requirement
