We are seeking a highly skilled RN Case Manager to join our team at University Hospital. As a key member of our care coordination team, you will play a critical role in ensuring the seamless transition of patients from hospital to home or other care settings.
Key Responsibilities- Conduct thorough assessments of patients' needs and develop personalized care plans to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate care and services, including home health, skilled nursing, and rehabilitation.
- Lead daily care coordination rounds to update plans and facilitate implementation.
- Initiate discharge plans, including early referrals to homecare, DME, and infusion services.
- Prepare patients and families for discharge, ensuring a smooth transition to the next level of care.
- Participate in quality improvement initiatives to reduce readmissions and improve patient satisfaction.
- Stay up-to-date with the latest clinical guidelines, policies, and procedures to ensure evidence-based practice.
- Bachelor's degree in Nursing or equivalent experience.
- Current State of Michigan Board of Nursing licensure.
- Minimum of three years of acute care RN nursing experience or 3 years care management of acute care patients.
- A minimum of one year of successful experience in at least one of the following classifications: nursing management (supervisor or higher), case management, or demonstrated advancement in clinical career ladder.
- Two years hospital discharge planning experience.
- Masters degree in nursing or other health-related field.
- Case Management Certification such as ACM or CCM.
- Excellent analysis and data management and PC skills.
- Ability to work in autonomous and self-directed manner.
Hours: 40
Shift: Day shift, 8am-4:30pm, with weekend and holiday commitments
Location: UMH Care Management
