Veterans Case Manager

Job Summary

We are seeking a highly motivated Case Manager to serve single veterans in interim housing at a San Carlos hotel. As a strong support and advocate for clients, you will help them attain stable housing and self-sufficiency.

Key Responsibilities
  • Develop and implement individual case plans for a caseload of clients
  • Provide on-going supportive and/or case management functions in accordance with the case/treatment plan
  • Communicate with probation officers, social workers, and family members as needed
  • Participate actively as a team member in supporting clients and the facility
  • Stay current and knowledgeable about external community resources, referrals, and service linkages
  • Provide crisis management for clients and make linkages for interventions as appropriate
  • Facilitate clinical or life skills groups
  • Maintain client and program records in accordance with applicable standards and regulations
Requirements
  • Bachelor's degree in social work, psychology, or related field
  • Minimum of two years' case management experience
  • Prefer a minimum of two years' experience in working with homeless Veteran population and/or individuals with serious mental health issues
  • Excellent communication and interpersonal skills
  • Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
