Key Responsibilities- Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Perform comprehensive assessments and document findings in a concise and compliant manner
- Facilitate agreement and implementation of the patient's plan of care
- Coordinate care as patients transition through different levels of care and care settings

Requirements- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions
- 1+ years of clinical experience in senior living, geriatric medicine, or home care
The salary range for this role is $88,000 to $173,200 annually, based on local labor markets, education, work experience, certifications, and other factors. UnitedHealth Group complies with all minimum wage laws as applicable.
