This role focuses on providing professional and non-professional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Clinical Nurse duties:
Key Responsibilities- Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
- Provides professional nursing care to patients. Requires a nursing license.
- Manages projects/processes, works independently with limited supervision, coaches and reviews the work of lower-level professionals, resolves difficult and sometimes complex problems.
- Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research and inpatient partners thus producing high-performing, patient-focused teamwork.
- Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope.
- Identifies high-risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
- Promotes shared decision-making with patients in regards to healthcare; educates the patient and/or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions.
- Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
- Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.
- Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment.
- Participates in satellite quality and safety initiatives as indicated.
- Bachelor's degree in Nursing (BSN). Associates degree considered with review of Nurse Executive Council.
- Registered Nurse (RN) license.
- Basic Life Support (BLS) certification.
- Specialty nursing certifications.
