Key Responsibilities:

  • Develop strong relationships with patients to understand their unique needs and preferences
  • Meet with patients during the check-in process to gather information and provide support
  • Call patients to schedule appointments, remind them of upcoming events, and provide education on their healthcare options
  • Order and schedule procedures, tests, and screenings as needed
  • Identify and track progress on important gaps in patient care, working closely with providers to ensure seamless coordination
  • Maintain accurate records of patient interactions and communicate effectively with providers using electronic health records
  • Educate patients on their healthcare options, insurance benefits, and common medical conditions, empowering them to make informed decisions about their care
  • Connect patients with community resources and support services to promote overall well-being
  • Collaborate with the healthcare team to ensure timely and effective follow-up care after hospital discharge
