Reports To: Administrator

Position Summary

The MDS Licensed Vocational Nurse plays a crucial role in supporting the Director of Nursing by ensuring that all documentation within the facility complies with federal, state, and certification standards. This position is responsible for coordinating the RAI process, ensuring that the MDS, CAAS, and Interdisciplinary Care Plan are completed accurately and on time.

Key Responsibilities
  • Assist in maintaining compliance with federal and state regulations.
  • Participate actively in regulatory surveys and assist in addressing any identified deficiencies.
  • Report audit trends to the Quality Assurance Committee.
  • Oversee the RAI Process from MDS completion to care plan development.
  • Track RAI processes, discharge/re-entry, and Medicaid documentation through relevant systems.
  • Ensure staff adherence to documentation standards.
  • Gather resident data and conduct interviews to uphold best practices as per the latest MDS guidelines.
  • Facilitate accurate Assessment Reference Date determinations to reflect patient care needs.
  • Provide interdisciplinary scheduling for MDS assessments and care plans in accordance with regulations.
  • Ensure required signatures are obtained for documentation.
  • Guide the interdisciplinary team in deciding on additional MDS assessments based on clinical criteria.
  • Respond appropriately to changes in patient conditions.
  • Coordinate patient care planning conferences and communicate outcomes to relevant parties.
  • Ensure that all MDS-related information is thoroughly documented.
  • Assist in monitoring care plan initiation for all patients upon admission.
  • Engage in daily interdisciplinary meetings to discuss MDS and care plan completion.
  • Act as a resource for MDS-related software issues and maintain proficiency in relevant programs.
  • Ensure proper Medicare coverage through consistent communication with specialists.
  • Submit final MDS and resident assessment data to state and federal agencies promptly.
  • Train staff on RAI processes and clinical systems.
  • Maintain organized records of MDS submissions and validation errors.
  • Uphold confidentiality of sensitive information.
  • Stay informed on industry changes and best practices.
  • Assist with compliance reviews and audits as necessary.
  • Coordinate monthly meetings for medical billing compliance.
  • Complete necessary documentation on relevant portals and communicate changes to ensure uninterrupted Medicaid payments.
  • Adhere to guidelines regarding residents' rights and ensure their safety.
Qualifications

Required:

  • Current LVN license.
  • CPR certification (30 days provided to obtain certification).
  • In-depth knowledge of the RAI Process, Medicare PPS, and Medicaid reimbursement.
  • Strong understanding of Quality Indicator processes.
  • Familiarity with OBRA regulations and Minimum Data Set.

Preferred:

  • Two years of nursing experience in long-term care or skilled nursing environments.
Physical Requirements

Ability to perform physical tasks such as pushing, pulling, lifting, bending, kneeling, and reaching, with or without reasonable accommodations.

Work Environment

Clinical setting.
