We are seeking a dedicated and compassionate Healthcare Specialist to join our team at TalentRupt Inc. as a Health Guide. In this role, you will lead and manage clinical assessments, provide crucial care coordination, and work directly with patients to ensure a comprehensive support system.

Key Responsibilities:

  • Lead and manage clinical assessments to ensure accurate diagnoses and effective treatment plans
  • Provide care coordination to ensure seamless transitions between healthcare services
  • Work directly with patients to understand their needs and preferences
  • Collaborate with community resources to ensure a comprehensive support system

Requirements:

  • Master's degree or higher in Primary Care or Value-Based Care
  • Active NP or PA-C license in WA
  • Coding experience preferred but not required
