Job Summary

University Hospitals is seeking a highly skilled Certified Medical Assistant to join our team as a Transitional Care Specialist. In this role, you will play a critical part in ensuring seamless transitions for patients from hospital to home, hospital to post-acute care, and post-acute care to home.

Key Responsibilities
  • Perform transition of care follow-up to patients discharged from a hospital, SNF, or acute care rehab
  • Coordinate continuity of care with patients and families following hospital admission discharge
  • Conduct transition of care phone calls to patients to review and educate on provider follow-up, diagnostic or procedure follow-up
  • Perform medication review with support and under the direction of the transitional care RN
  • Provide patients/family/caregiver with information on diagnosis and self-care management
  • Refer to interdisciplinary team members if patient meets the criteria
  • Establish discharge referrals and assist in scheduling any required follow-up with providers, community supports and services
  • Escalate patient concerns to transitional care RN as needed
  • Demonstrate a thorough understanding of, and ability to connect patients with, available resources in the Northeast Ohio community focused on improving the health, wellness, and overall quality of life for a patient
  • Communicate with Home Health Care agencies or community agencies utilized by the patient
  • Provide ongoing support as needed or refer to RN based on patient's condition and level of acuity
  • Document within the medical record per established guidelines
  • Assist manager with developing and facilitating process improvement initiatives
  • Facilitate unit operations by managing patient flow
  • Work as a healthcare advocate for patients and communicate with other healthcare providers to accurately update documents and schedule appointments
  • Assist patients in removing transportation and other barriers that could prevent patients from getting healthcare
Requirements
  • Graduate of an accredited Medical Assistant Program (Required)
  • 2+ years Of progressive work experience in a healthcare environment. Clinical based knowledge is a must. (Required) and
  • Experience working multiple healthcare data sources that include Electronic Medical Record applications(s), registration and scheduling systems. (Required) and
  • Experience facilitating telephonic patient outreach. (Required)

  • Proficiency in provider office workflow. (Required proficiency)
  • High level of integrity and ability to maintain confidentiality of sensitive data. (Required proficiency)
  • Understanding of value-based care program trends and strategies. (Required proficiency)
  • Excellent verbal and written communication skills. (Required proficiency)
  • Proven time management and organizational skills. (Required proficiency)
  • Demonstrated excellence in customer service with all stakeholders (Patient, employer staff, providers, etc.). (Required proficiency)
  • Demonstrated ability to apply concepts, utilize sound judgment and work independently. (Required proficiency)
  • Ability to work within a team environment. (Required proficiency)
  • Knowledge of healthcare resources, key quality measures and benefits. (Required proficiency)
  • Ability to handle multiple priorities within a fast-paced environment. (Required proficiency)
  • Ability to problem-solve and keep composure and professionalism during high stress situations. (Required proficiency)
