Our focus is on keeping our seniors healthy, and we achieve this by managing patients within a global capitation budget provided by Medicare Advantage plans.

About the Role

The Cardiologist will partner with our team-based care model to support our value-based care provider focused on delivering high-quality care for our patients. Our care team consists of physicians, advanced practice professionals, medical assistants, behavioral health support, quality-based coders, referral coordinators, and more.

Our approach allows us to provide an unmatched experience for seniors, with a focus on concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships. This robust support enables our PCP to see fewer patients and spend more time caring for patients.

Responsibilities
  1. Evaluates and treats center patients in accordance with standards of care.
  2. Assists in the coordination of patient services, including specialty referrals when appropriate, cardiology subspecialists, and vascular surgeons.
  3. Acts as an active participant and key source of medical expertise with the care team.
  4. Partners with primary care physicians and nurse practitioners in a tone of cooperation in practice by displaying a professional and approachable demeanor.
  5. Completes all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation.
  6. Follows policy and protocol defined by Clinical Leadership.
  7. Meets with primary care physicians, including nurse practitioners, and market medical director to discuss quality of care, review of outcome data, and medical records issues.
  8. Supports our longitudinal care model for heart failure with a focus on disease management and prevention.
  9. Supports readmission reduction initiatives for heart failure and CAD.
  10. Creates a practical framework for upstream stroke prevention.
  11. Helps coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
  12. Interprets and documents in detail test results to determine how effectively the heart and or cardiovascular system is functioning, including Echocardiograms and stress tests.
  13. Coordinates patient care with cardiology subspecialists, including interventional, EP, Thoracic, and more.
Qualifications

Graduate of an accredited MD or DO program of an accredited university.

Current and unrestricted medical license or willing to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.

Board Certification in Cardiology.

Active and unrestricted DEA license.

Medicare Provider Number.

Medicaid Provider Number.

Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.

Fully engaged in the concept of team-based care model.

Willingness and ability to learn/adapt to practice in a value-based care setting.

Superior patient/customer service.

Basic computer skills, including email, EMR, and more.
