**Job Summary:**

  • We are seeking a dedicated and compassionate Care Manager to join our team in Fairfax.
  • The ideal candidate will have previous experience working with seniors, excellent communication skills, and a passion for delivering high-quality care and services.

**About the Role:**

You will be responsible for building meaningful relationships with our residents and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.

**Responsibilities:**

  • Provide high-quality care and services to a consistent group of residents and their families.
  • Build meaningful relationships with residents and their families through regular interactions and activities.
  • Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Responsible for designated group of residents during shift, knowing where they are and physically checking on them throughout the shift.
  • Partner with community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
  • Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
  • Invite, Encourage, Assist (IEA) residents in life skills and other life-enriching activities as indicated on ISP and demographic profile.

**Requirements:**

  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility to perform various duties in service to residents, and ability to work in a team environment are keys to success.
  • High School diploma/GED accepted and may be required per state regulations.
  • In states where appropriate, must maintain certifications.
  • Must be at least 18 years of age.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in resident's best interest.
  • Possess written and verbal skills for effective communication and a level of understanding.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.

  • Competitive salary: $45,000 - $60,000 per year based on location, skills, experience, qualifications, and certifications/licenses (where applicable).
