Nurse Practitioner

Job Summary:
PruittHealth Premier Plan is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide holistic, patient-centered care to adults and seniors, working collaboratively with physicians and other healthcare professionals to promote wellness and manage chronic conditions.
Key Responsibilities:
- Conduct comprehensive physical exams and diagnose chronic diseases in adults
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination
- Participate in Quality Assurance Performance Improvement (QAPI) activities to drive quality and safety initiatives
- Stay up-to-date with the latest medical knowledge and best practices through ongoing education and professional development
Requirements:
- Master's degree in Adult or Family Nurse Practitioner program
- Minimum 2 years of clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting
- Current and unrestricted RN and NP licensure in state of practice
- Board certification by AANP or ANCC in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care
