Patient Care Coordinator Job Summary

We are looking for a Patient Care Coordinator to join our team at Walgreens. As a key member of our pharmacy staff, you will work closely with patients and healthcare professionals to coordinate care and promote health and wellness.

Responsibilities:

  • Coordinate patient care activities and communicate with healthcare providers.
  • Ensure accurate and timely processing of prescriptions.
  • Provide patient education and counseling on medication use and disease management.
  • Stay up-to-date on the latest developments in pharmacy practice and technology.

Requirements:

  • BS in Pharmacy or related field.
  • Current pharmacist licensure in the states within the district.
  • At least 2 years experience as a pharmacist in a retail setting.

Salary Range:$70,000 - $115,000 per year
