Job Description

We are seeking an experienced Patient Flow Coordinator to join our team in Patient Flow Operations. As a key member of our department, you will play a vital role in ensuring seamless patient transitions throughout our healthcare system.

Key Responsibilities:

  • Central Bed Management: Work closely with physicians to place inpatient/observation patients in the most appropriate units, ensuring efficient use of bed capacity.
  • Transfer Center: Facilitate direct admits and patient transfers between Prisma Health facilities, external health care facilities, and physician practices, promoting safe and timely care transitions.
  • Care Coordination: Collaborate with various teams, including Emergency Department, inpatient units, Administrators on Duty, and Environmental Services, to optimize patient throughput and ensure high-quality care.
  • Process Improvement: Identify opportunities for process enhancements and implement standardized workflows to drive efficiency and quality improvements.

Requirements:

  • Education: Graduate of an accredited school of Nursing program recognized by the SC State Board of Nursing; Bachelor's degree in Nursing preferred.
  • Experience: Three (3) years Acute Care Nursing experience.
  • Licensure: Licensed to practice as a RN in South Carolina.
