Nurse Practitioner

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Sky View Rehabilitation and Health Care Center. Our 192-bed Skilled Nursing Facility is located in Croton-on-Hudson, NY, and we provide a luxurious and hotel-like environment for our residents.
Job SummaryAs an LPN at Sky View, you will play a vital role in delivering high-quality patient care and supporting our residents in their rehabilitation journey. Your responsibilities will include:
- Providing daily support to the unit leadership and responding to resident and family needs within the LPN scope of practice
- Using the nursing process to set priorities and deliver quality direct resident care
- Performing and administering standard nursing treatments and medications, and observing residents for desired or unwanted effects
- Communicating and collaborating with the care team to revise and update the Nursing Care Plan
- Monitoring and documenting vital signs, treatments, and resident responses
- Preparing and organizing the delivery of care for resident CNA assignments and activities
To be successful in this role, you will need:
- Graduation from an approved school of nursing
- Current licensure and registration in good standing in New York as an LPN
- Dedication to providing extraordinary, clinically sound, and compassionate care
- A spirit of cooperation, caring, and enthusiasm to create a positive atmosphere for rehabilitation and optimal resident health
- Excellent communication, customer service, and organization skills
- Ability to work well under pressure and harmoniously with all staff
