Estimated Salary: $70,000 - $90,000 per year

Job Responsibilities
  • Develop and implement patient care plans in collaboration with our interdisciplinary team
  • Provide emotional support and counseling to patients and their families
  • Maintain accurate and timely documentation of patient interactions and progress
  • Promote patient autonomy and independence
Requirements
  • Masters in Social Work from an accredited graduate school
  • Minimum one year of experience in a healthcare setting as a Medical Social Worker
  • Licensed as a Medical Social Worker by the state in which you work
  • Valid driver's license and ability to spend 20% of your day driving to/from patient locations
