We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated caregivers. As a home health LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a home care setting
