Job Opportunity:

Crescent Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to provide patient care in the Aiken Area. As a member of our team, you will play a vital role in delivering high-quality care to our patients and their families.

About the Role:

As a PRN LPN, you will be responsible for providing direct patient care under the supervision of a registered nurse. Your duties will include assessing and evaluating patient status, following the plan of care, and working collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:

• Assess and evaluate patient status, regularly reevaluating patient and family/caregiver needs
• Provide focused physical assessments, considering physical, emotional, and spiritual aspects
• Attend and provide support at the time of death, as ordered by the physician and per local regulations
• Maintain regular communication with the attending physician and RN Case Manager, reporting changes in patient condition and obtaining/receiving physicians' orders as required
• Monitor and maintain documentation of clinical records, ensuring compliance with regulatory standards of timeliness, accuracy, and completeness

Requirements:

• Graduate of an Accredited Practical Nurse or Vocational Nurse Program
• Current unencumbered license in the state of practice as an LPN
• Minimum three (3) years' experience as LPN in Community Health, Medical surgical/Acute care setting required
• Hospice/home health experience preferred but not required
• Computer proficiency in typing and various programs, including background in EMR
• Current CPR certification
